PARENT’S CONTRACT
I whole heartedly agree to abide by the school policies and regulations written in the Student’s Handbook, thus giving my full support and cooperation in all activities and projects of the school. During such activities, I agree that the school having taken all precautionary measures will not be held responsible for any untoward incident beyond its control. Moreover, I pledge to give time to attend all Parents Teachers Fellowship, activities and individual parent teacher meeting. I will give my full support on programs that require fees and my presence.
PERSONAL INFORMATION AGREEMENT
By signing this form, I/we, the undersigned student and/or parent(s)/legal guardian(s), acknowledge and agree to the following in compliance with the Data Privacy Act of 2012 (RA 10173):

Consent to Process: I/we voluntarily provide and authorize Antipolo Faith Christian School Incorporated to collect, store, and process personal and sensitive information (such as name, address, religious affiliation, health records, and academic history) for admission, enrollment, and legitimate educational purposes.

Reporting: I/we understand that the School is mandated to share student data with the Department of Education (DepEd) via the Learner Information System (LIS) and other regulatory agencies as required by law.

Media Release: I/we grant permission for the School to use the student’s photograph or video recordings in official school publications, social media pages, and promotional materials for the purpose of celebrating school achievements and Christian community life.

Retention: I/we understand that while most records are kept only as long as necessary, certain academic records (e.g., Form 137/SF10) will be retained permanently as required by educational regulations.

Rights: I/we am/are aware of my/our rights to access, correct, or object to the processing of my/our data by contacting the School’s Administrative Officer.
